Patient Portal
The patient portal allows you to view your patient records, message with your doctor, check-in online before your appointment, and more.
You will need the Activation Code sent to you via email (check your Spam/Promotions folder) or given to you in your After Visit Summary. If you need help finding the Activation Code, call our office for assistance.
